BY PURCHASING AN ONLINE ZEN MARCH BREAK AND/OR PA/PD DAY CAMP MEMBERSHIP YOU ARE AGREEING TO THE FOLLOWING TERMS:

PA/PD DAY CAMP Cancellation  Policy: All cancellation requests must be received in writing.  Full refund less a $10 cancellation fee on all cancellations prior to the start of camp.  All cancellation requests made after we start the first day of camp or no shows are non refundable.

MARCH BREAK CAMP Cancellation Policy: All cancellation requests must be received in writing.  Full refund less a $50 Cancellation fee on cancellations made before February 1st.  Cancellations made between February 1st and the first day of camp are subject to a 50% cancellation free.  All cancellation requests made after we start the first day of camp or no shows are non refundable.
 

Kids camps, martial arts and/or fitness programs require physical exertion which may be strenuous and cause physical injury, and I am fully aware of the risks and hazards involved. I hereby represent that my child is physically fit to receive and participate in the prescribed course of instruction. I acknowledge that I have been advised to consult with a physician prior to and regarding my child's participation in the camp and that my physician has confirmed to me that my child has no medical condition which would affect their full participation in the camp.
 

In consideration of my being permitted to participate in the camp, I for myself, my heirs, executors, administrators, successors, assigns and anyone else who may claim on my behalf hereby waive any and all claims, liability and damages I may now or in the future have against Zen Mixed Martial Arts (zen), its directors, officers, instructors and staff and all persons acting under its authority and their respective heirs, executors, administrators, successors, and assigns, arising by any means whatsoever, including, but not limited to death, injury, damages to or loss to my person or property of any kind whatsoever, no matter how caused arising from or in connection with my participation in the summer camp, however caused
 

I further hereby release, remise and forever discharge Zen, its owners, directors, officers, partners, instructors and staff and all persons acting under its authority and their respective heirs, executors, administrators, successors, and assigns from and against all demands, claims, actions, damages, costs and expenses arising from or with respect to death, injury, damages to or loss to my person or property of any kind whatsoever, no matter how caused, arising from or in connection with participation in the camp whether or not caused by, in whole or in part, negligence or gross negligence, breach of contract, breach of any statutory or other duty of care, including any duty owed under the Occupiers' Liability Act (Ontario).
 

I will hold harmless and indemnify Zen, its directors, officers, instructors and staff and all persons acting under its authority and their respective heirs, executors, administrators, successors, and assigns from any and all liability for any damage or injury to the property or persons of any third party resulting from the Releasor's involvement in or presence during camp.
 

We will comply with all the rules and regulations of Zen.
 

Failure to attend the summer camp does not relieve me of the obligation to pay the full tuition for the program.
 

If the student and/or co-signer wish to cancel this agreement on the stated terms set out, they may do so by sending a written notice to Zen at info@zenmma.ca
  
I confirm that the provisions in this Agreement constitute the entire agreement between the parties hereto and supersede all prior communications, proposals, representations and agreements, whether oral or written with respect to the subject matter of this Agreement.

ZEN MIXED MARTIAL ARTS INC. (ZEN) or any party designated by ZEN may photograph or film your child while attending Camp and use of any and all such photos, video footage and/or video streaming for promotion, sales, publicity and advertising purposes for all media, including, but not limited to, the Internet.
 

I acknowledge having been given full opportunity to read this agreement before registering, having full opportunity to ask questions regarding the activity, and having read and understood this agreement (or where the participant is a minor, having his/her parent or legal guardian read the agreement and explained it to the participant) and received a copy of this Agreement.
 

I understand my rights and obligations as stated above. I confirm that I am the full age of eighteen years (unless otherwise indicated below) and that I have read and understood this release and waiver of liability and I am aware that by signing this Agreement I am waiving certain legal rights, including the right to claim damages which I or my heirs, executors, administrators, successors or assigns my have against Zen.
 

If you cancel this contract, Zen has 15 days to refund your money.
